Thank you for your interest in HFS Andrology Laboratory. We are your one stop shop for all of your male factor patients. If you would like to refer a patient to our laboratory, please complete and sign the test requisition form below. Please remember to include your fax number so our laboratory can send your office the test results. Your patient will need to call 713-512-7600 to schedule an appointment and are responsible for lab payment. Please do not hesitate to contact us with any questions.
